EBI, LLC warrants to the patient that products will be free from defects in materials and workmanship for a period of three (3) years following the date of delivery of the product to the patient.

EBI will repair or replace, at its option, any EBI® manufactured or provided product found to be defective in materials or workmanship and covered by this Limited Warranty. Normal wear and tear and damage caused by accident, misuse, alteration or other external forces are not considered to be defects.

Defective products covered by this Limited Warranty must be returned to EBI in accordance with its return authorization policy and procedure. Please contact the EBI® Customer Care team at 800-526-2579 to obtain a return authorization and further instructions for such return.

THIS LIMITED WARRANTY IS EXCLUSIVE AND IS MADE IN LIEU OF ANY AND ALL OTHER WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WITHOUT LIMITATION ANY IMPLIED WARRANTY OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE, ALL OF WHICH ARE HEREBY DISCLAIMED.

Repair or replacement is the patient’s sole and exclusive remedy, and the sole and exclusive obligation and liability of EBI, and under no circumstances shall EBI be liable for compensatory, special, consequential or incidental damages except as may be required by applicable law.

This Limited Warranty applies only to the patient for whom the product was initially prescribed and is not transferrable.

The terms of this Limited Warranty cannot be expanded or otherwise altered except in writing approved by a duly authorized officer of EBI.
